血液透析患者的营养状况分析

　　【摘要】 目的 了解不同年龄、不同透析年限的MHD患者的状况差别。方法 将80例MHD患者按不同透析年限分为3年以下组、3～6年组、6年以上组。其中3～6年组的患者再按年龄不同分为40岁以下组、40～60岁组、60岁以上组。所有患者检测血清白蛋白(AlB)、转铁蛋白(TF)、前白蛋白(PA)、体重、肱三头肌皮肤皱褶厚度(TSF)、上臂围(MAC)、上臂肌围(MAMC)等指标。结果 (1)AlB、TF、PA3个生化指标在透析年限不同的3个组间做两两比较，差异有显著性(P<0.05)，3个指标均表现为6年以上组<3～6年组<3年以下组。体重、MACA在各组间做两两比较，差异无显著性(P>0.05)。(2)不同年龄组患者各指标比较结果表明，60岁以上组的AlB、TF、PA、MACA低于其余两个组(P<0.05)。结论 MHD患者中，年龄越大，接受透析时间越长，营养不良的程度越严重。

　　【Abstract】 Objective To know the difference of the nutritional condition among the different age patients and the different patients who were treated for different time.Methods Eighty MHD patients were divided into3groups by treated time as follows:the group of under3years，the group of3～6years，and the group of beyond6years.The patients in group of3～6years were divided into another3groups by age.They were the group of under40years old，the group of40～60years old，and the group of beyond60years old.AlB，TF，PA，body weight，TSF，MAC，MAMC of every patients were determined.Results (1)In the group which divided by treated time，the statistic signifˉicant different value was less than0.05when compared each other with AlB，TF and PA，but the value was more than0.05when compared each other with weight body and MACA.(2)In the group which divided by age，AlB，TF，PA，MACA of the group beyond60years were less than the other groups(P<0.05).Conclusion In MHD patients，the older the age was and the longer the hemodialysis time was，the more serious the malnutrition was.

　　近30年来，随着透析技术的不断发展，透析疗法已成为终末期肾脏替代治疗的主要方法之一。在维持性血液透析(MHD)患者中，营养不良问题一直受到临床肾病医生的重视。营养不良与MHD患者的发病率和病死率升高密切相关，是影响患者预后及生存质量的重要因素 [1～3] 。本研究对在我院接受血液透析治疗的80例患者的营养状况进行了分析，旨在了解不同年龄、不同透析年限的患者的营养状况的差别。

　　1 对象与方法

　　1.1 研究对象 选取在我院接受MHD患者80例，年龄29～76岁，排除感染及其他活动性疾病。根据研究目的，将80例患者分别按透析年限和年龄的不同进行了分组。分组情况如下:(1)按透析年限不同分组共分3组。3年(包括3年)以下组:20例，男14例，女6例;3～6年组:35例，男18例，女17例;6年以上(包括6年)组:25例，男12例，女13例。(2)将透析年限为3～6年的患者再根据不同年龄分3组。40岁以下(包括40岁)组:8例，男4例，女4例;40～60岁组:15例，男8例，女7例;60岁以上(包括60岁)组:12例，男6例，女6例。

　　1.2 研究方法

　　1.2.1 检测指标及检测方法

　　1.2.1.1 生化指标 血清白蛋白(AlB)采用Hitachi7170全自动生化分析仪检测;转铁蛋白(TF)、前白蛋白(PA)采用BN100特定蛋白分析仪(免疫散射比浊法)检测。

　　1.2.1.2 人体测量指标 包括体重、肱三头肌皮肤皱褶厚度(TSF)、上臂围(MAC)的测量。其中TSF用游标卡尺测量，MAC用软尺测量。根据TSF和MAC计算出上臂肌围(MACA)。MACA=MAC(cm)-3.14×TSF(cm)。
